PROBLEM: We referred to the woman wanting to bury her dead husband as being an 81-year-old woman. In fact, the husband was 81, and no age was given for the woman.
SOLUTION: Be sure to double-check all information sets (like names, addresses, ages, etc.) BEFORE we start writing to make sure we know what is what; and double-check again AFTER writing to make sure what we wrote down was in fact correct.

PROBLEM: We said the zoning board made its decision last week; in fact, it made its decision last night about what to do with the body of the man who died last week.
SOLUTION: Again, before writing make sure you have the facts straight and that you're not confusing anything, and then after you write go through your work fact-by-fact to make sure you presented the correct information correctly.
